This policy describes how the Neurologic Disease Foundation protects, collects, uses, and shares information you provide to us.
Although the Foundation is not a Covered Entity as described in federal law and regulation, we are sensitive to the issues regarding protected health information (PHI) and personal information that you provide to us. Therefore, the Foundation handles your personal information very carefully.
Information We Collect
NDF collects limited information you submit to us, primarily in the registration process for our educational programs. We collect name, address, telephone number, email address, associated caregivers, your area of interest (i.e., multiple sclerosis or Parkinson disease) and past NDF programs you have attended.
Information We Do NOT Collect
NDF does not seek, nor collect, further information regarding your personal medical status or history. Individual presenters who are healthcare providers may have additional information about you, but the NDF does not seek, nor collect, that information.
How We Use Your Information
NDF uses your information for internal purposes relating to registration and sending notices regarding upcoming programs.
From time-to-time medical providers make contact information or label information available to the Foundation. This is done to enable their patients to participate in NDF programs. NDF does not retain this information unless you subsequently register for a program.
How We Share Your Information
NDF does not share information with other organizations or sponsors of its programs. It does not sell your information. It does not provide information for marketing purposes to outside organizations.
NDF has no specific medical information about you to share or disclose. We do not disclose your area of interest.
